The study has been launched by Active Black Country which is the sporting arm of the Local Enterprise Partnership.

A spokesman for Active Black Country said: "We are looking to ascertain the impact of social distancing on the activity levels of people living and working in the Black Country.

"This survey includes questions that pertain to activity habits before and during the lockdown, and seeks to understand if and how any lifestyle changes adopted during this period might be retained."

All respondents will be entered into a prize draw. The winner will receive £200 in physical activity vouchers.
