It was defender Liam Ridgewell who swept in the leveller, to ensure Alex McLeish's men bounced straight back from the end of their 15-game unbeaten run at Chelsea in midweek.
It cancelled out Jermain Defoe's 69th minute goal f0r Spurs, who were left to rue another late breach - after a stoppage time goal saw Leeds snatch a replay against them in the FA Cup last weekend.
An even first-half saw Birmingham striker Christian Benitez fire meekly at goalkeeper Heurelho Gomes after turning Michael Dawson, while Peter Crouch screwed a shot wide of the post when well placed.
Defoe finally broke the deadlock in the second-half, after Gareth Bale's run to the byline and cross was flicked on by Crouch into the path of the striker, who smashed his shot past goalkeeper Joe Hart.
That appeared to be that but Blues had other ideas, getting their reward at the death.
James McFadden's dinking run and cross saw Jerome muscle in to head the ball across goal into the path of Ridgewell, who forced the ball home for a welcome draw.
