Hornets goalkeeper Scott Loach had an inspired game until then, to frustrate Birmingham in their quest for promotion from the Coca-Cola Championship.
Blues were entirely dominant at Vicarage Road, but were unable to find a way past Loach, so after a poor start to the second half Alex McLeish made what proved to be an inspired substitution.
He brought on Jerome in place of O'Connor, paying off almost immediately when the former Cardiff striker handed Birmingham a crucial lead.
Skipper Damien Johnson played him in and Jerome cut inside to score a deflected effort, which sailed into the far corner of the Watford net to send the visiting supporters wild.
