A probe is being carried out on the tragedy this summer and comes less than a month after it emerged the trust which runs the hospital is to be prosecuted over the death of 66-year-old Gillian Astbury.

She died at the hospital in April 2007 after not receiving insulin. It led the HSE to launch criminal proceedings against the Mid Staffordshire NHS Foundation Trust.

Now it has emerged another investigation has been launched. Kevin Hegarty, ofthe HSE, said: "HSE can confirm it was notified of the death of an elderly patient at Stafford Hospital in July 2013.

"We have accepted that it meets our published investigation criteria."
