The A41 Holyhead Road was closed for nearly two hours following a collision between a car and a 4x4 vehicle just past Wergs Road, near The Foaming Jug pub.
The air ambulance from RAF Cosford flew one of the injured, a 57-year-old woman with severe chest and facial injuries, to Selly Oak Hospital. The victim, who also suffered a broken arm, had been a front-seat passenger in the off-road vehicle.
West Midlands Ambulance Service spokesman Chris Kowalik said: "There was extensive damage to the 4x4 vehicle which was carrying a couple in their 50s.
"The woman was given pain relief at the scene and flown to Selly Oak Hospital. The hospital was on alert."
The 53-year-old male driver appeared to be unhurt but was taken by land ambulance to New Cross Hospital as a precaution.
The driver of the car, also male, was out of his vehicle when crews arrived.
Firefighters cut the roof and doors off the vehicle to free the couple. Station officer Paul Baker said the impact of the crash demolished part of a stone wall.
The accident just after 12.35pm on Saturday.
