Little Tyler Nicholls was left with severe cuts and bruises following the drama, which happened at a roundabout in Bushbury Lane, Oxley.
The 21-month-old was travelling in the back of his grandmother Janet Timmins' Peugeot 306 with his mother Samantha Timmins sitting alongside him.
Samantha, a 25-year-old Pizza Hut worker of Cadle Road, Bushbury, told today how she screamed as he fell out of the door. "I can't believe this happened on the one day the child locks were not on," she said. "One second he was there and the next he was gone.
"His head was pouring with blood and he was black and blue. He had to spend the night at New Cross Hospital."
The accident happened at 8.30pm on Monday (8). Grandmother Janet, 52, added: "It is a miracle he was not more badly hurt."
