Wolverhampton teenager Chelsea Clark told of threat to self harm

A 13-year-old schoolgirl found hanged in her bedroom had previously taken an overdose and told healthcare workers she would try to harm herself again, a review into her death has revealed.

Chelsea Clark had said she would 'self harm or worse' if allowed to return home after an apparent suicide attempt, a serious case review has found.

Chelsea, a pupil at Wolverhampton Girl's High, was found dead at her home in Finchfield, Wolverhampton, on June 29, 2011.

A probe has concluded it is not possible to say the teenager's tragic death was preventable.

A 35-point action plan has now been drawn up outlining recommendations for all the agencies involved.
